Child Requirments: Prior to admission, all necessary forms must be completed and submitted to the day care including, but no limited to; a contract, personal information questionnaire, and a medical authorization. Each child shall have received a medical examination by a physician within ninety days prior to admission. Each child must also have had a lead test per the new public health law passed December 1993, and appropiate dental health exams. A written statement signed by the physician administering the medical exam shall be furnished to the day care giving assureance that the child: 1. Is free from contagious or communicable disease; and 2. Has received prohylaxis against diptheria, tetanus, polio myelitis, pertussis, measles, rubella, mumps, and haemophilus influenza (Hib) appropriate to hes/her age except where there are medical or religious contraindications. Lively Bunch Day Care does not discriminate on the basis of sex, race, color, religion, or national origin in the admission of children. Each child should bring an extra set of clothes in case of an accident. Diapers are to be provided by the parents for their infant. In the winter time, children should come equipped with mittens, hats, and snow pants. |
